Advanced Care Planning for End-of-Life Patients
The Lawns has many years of experience providing high-quality End-of-Life (EOL) and Palliative Care in Plymouth, Devon. We share the ethos of Dame Cicely Saunders and recognise the importance of ensuring our residents die peacefully, comfortably, and in an environment where they feel at home.
Advanced Care Planning (ACP) conversations are vital to our care planning process when someone is approaching the end of their life. These ACP conversations will be carried out by our managers, Sam Burdekin and Lucie Burdekin. They will have these discussions with the resident and their loved ones to ensure we understand each individual’s specific wishes and create a comfortable and calming environment during their last days of life.
St Luke’s Six Steps EOL Care Home
The Lawns is in the process of becoming accredited as a St Luke’s Hospice Six Steps Care Home. Three of our nursing team, including Sam Burdekin, Registered Manager, have completed the Six Steps EOL care programme with St Luke’s Hospice Plymouth and are now EOL Champions. Feel free to read more about the Six Steps Programme. St Luke’s provide high-quality end-of-life care and support both in the community and at their specialist hospice care unit.
The Six Steps + Programme is a series of workshops developed by the St Luke’s education team. The programme serves as a toolkit for Care Homes and agencies providing palliative care, helping them meet best practices and CQC (Care Quality Commission) standards. As EOL Care Champions, we are committed to sharing our knowledge and skills with peers and colleagues.
